B. END USER LICENSE AGREEMENT
Please read the following End User License Agreement (“EULA”), including all disclaimers and terms and conditions of use below, carefully before using the OneDashboard. By using or accessing this OneDashboard, you acknowledge that you have read, understood and accepted the disclaimers and terms and conditions of use contained in this EULA. If you do not accept any of these disclaimers or terms and conditions, you must not use the OneDashboard.
Information in OneDashboard was been developed by UHN.
1. DISCLAIMER: THE UNIVERSITY HEALTH NETWORK MAKE NO WARRANTIES OR REPRESENTATIONS AS TO THE ACCURACY OF THE INFORMATION PROVIDED. THE INFORMATION CONTAINED IN OR PRESENTED FROM THE OneDashboard IS EXCLUSIVELY INTENDED FOR REMOTE MONITORING PURPOSES AND COMES WITHOUT ANY REPRESENTATION OR WARRANTY OF ANY KIND, EXPRESSED OR IMPLIED. Under no circumstance shall UHN (including any staff or contractors) be held liable for consequences caused by or deriving from the use of the OneDashboard or any information contained in the OneDashboard. UHN is not liable for damages arising from use of the OneDashboard, including any that may arise from any defects or viruses (or similarly disruptive elements) caused from downloading or using the OneDashboard, or from websites (via hyperlinks) to which references are made in the OneDashboard, nor from information provided in the OneDashboard or these referred websites. In no event shall UHN be liable for direct, indirect, consequential, special, exemplary, or other damages related to your use of the OneDashboard (including any loss of data), regardless of how arising or the theory of liability whether arising in contract, tort, negligence or otherwise. Computer devices may malfunction from time to time and any unsaved data may be lost.
3. Governing Law and Jurisdiction. This EULA shall be interpreted under, and any action or claim arising from or related to the OneDashboard shall be brought in the courts of, and governed exclusively by, the laws of Ontario, Canada and the applicable laws of Canada applicable therein, without regard to its conflicts of laws principles. Unless prohibited by applicable law, you expressly waive the right to participate in a class action proceeding.
4. Changes and Updates. UHN reserves the right to update or change information contained in the OneDashboard without prior notice or prior acceptance.
5. Purpose of OneDashboard. The purpose of OneDashboard is to support clinical engagement and research participant management by consolidating patient-reported outcomes and app data into a centralized platform. This platform is currently intended to be integrated with non-SaMD (non-Software As A Medical Device) Center products that require clinical engagement or research participant management. The degree of intended use varies depending on the product requirements that are to be integrated with the system. At a base level, it is not intended to diagnose or treat patients directly, but rather to facilitate monitoring, coordination, and oversight by clinicians and research staff. As such, the OneDashboard is not a medical device. Nothing contained in the OneDashboard should be construed, directly or indirectly, as the practice of medicine or providing medical advice or medical services by UHN. The OneDashboard is not intended to diagnose, cure, mitigate, treat, or prevent disease or other conditions, it is simply intended to support with user monitoring in a research capacity.
6. Not Medical Advice. OneDashboard is not classified as a medical device and is not intended to diagnose, treat, cure, or prevent any disease or medical condition. It is designed to support clinical engagement and research participant management by consolidating patient-reported outcomes and app data into a centralized platform.
While OneDashboard may display alerts or notifications to assist clinicians in monitoring patient-reported data, these alerts are informational only and should never replace or override the clinical judgment of the patient’s medical team. All decisions regarding diagnosis, treatment, or care must be made by qualified healthcare professionals based on their expertise and the full clinical context. OneDashboard is not a substitute for professional medical advice, diagnosis, or treatment.
7. No Regulatory Approval. The OneDashboard has not been evaluated by Health Canada, the United States Food and Drug Administration (“USFDA”) or any other domestic or foreign regulatory agency and is not approved to diagnose, treat, cure or prevent any disease or for the prescription of any medication or treatment. While the information contained may be of use in other jurisdictions, these materials were provided and designed for use in Canada and you are solely responsible for ensuring suitability and permitted use in other jurisdictions.

9. Express Consent to Receiving Electronic Messages and Upgrades from UHN. By using the OneDashboard, you hereby expressly consent to receiving electronic messages from UHN, including alerts and reminders, information, notifications, requests for your feedback, and other communications from UHN and as well as any applicable software updates or upgrades.
C. COLLECTION OF DATA
1. Transmission of non-identifying data. The OneDashboard collects non-identifiable personal data (which is transmitted back to UHN) through the OneDashboard regarding the use of the OneDashboard in order to better understand use of the OneDashboard and to help UHN identify areas for improvement. If you do not consent to the transmission of this data, then you must not use the OneDashboard.
2. Transmission and Storage of Identifying Data and PHI. The OneDashboard also collects the user’s personal information and/or personal health information (“PHI”) including name (first name or pseudonym), province or territory (optional), email address, year and month of birth (optional), cell phone number (optional), and self-reported symptoms. This information is stored on the OneDashboard and also transferred to the OneDashboard server. Information stored on the OneDashboard Server is accessible by authorized administrators at these sites. More information can be found in the OneDashboard Privacy Policy, which is accessible below.
3. DISCLAIMER. THE OneDashboard TRANSMITS YOUR PERSONAL INFORMATION AND PERSONAL HEALTH INFORMATION OVER THE INTERNET. INFORMATION TRANSMITTED OVER THE INTERNET MAY BE AT RISK OF INADVERTENT LOSS OR THEFT. UHN CANNOT GUARANTEE THE SECURITY OF ANY INFORMATION TRANSMITTED TO OR FROM THE OneDashboard. BY USING THE OneDashboard, YOU AGREE THAT YOUR ACCESS AND USE OF THE OneDashboard AND THE CONTENT, INCLUDING BUT NOT LIMITED TO PERSONAL INFORMATION AND PERSONAL HEALTH INFORMATION, IS ENTIRELY AT YOUR OWN RISK AND LIABILITY.
D. LIMITED LICENSE.
Through your website access, provided that you comply with the terms of this EULA, you are acquiring, and UHN grants you a personal, limited, non-exclusive and non-transferable license to use the OneDashboard on your device for personal, non-commercial use and subject to any usage rules set forth by the distributor. This EULA does not entitle you to receive from UHN hard-copy documentation, support, telephone assistance or enhancements or updates to the OneDashboard.

Privacy Policy
This privacy policy governs your use of the software application OneDashboard (“OneDashboard”) that was created by the University Health Network (UHN), Centre for Digital Therapeutics. The Centre for Digital Therapeutics is a digital health research and development group that is hosted at UHN.
OneDashboard is a centralized digital platform designed to support clinical engagement and research participant management. It consolidates patient-reported outcomes and app-generated data into a single interface, enabling clinicians and research staff to monitor trends, review participant information, and receive alerts that assist in oversight and coordination.
OneDashboard is not a medical device and does not provide diagnostic or treatment capabilities. While it may display alerts based on patient-reported data, these alerts are intended for informational purposes only and should never replace or override the judgment of the patient’s medical team. The platform is currently intended for integration with non-SaMD (non-Software as a Medical Device) products that require clinical engagement or research participant management.
The goal of OneDashboard is to streamline data access and improve communication between clinicians and research teams, while maintaining strict adherence to privacy and security standards. It is not intended to replace your current care, but to supplement it with additional support.
Our Commitment to Privacy
This Privacy Policy provides you with information on how OneDashboard collects, manages, and protects your data (including any Personal Health Information) which you share within the OneDashboard. It further describes which particular data may be shared with others, what data will not be shared, and how you are able to control and manage this sharing.
When users choose to create an account on a OneDashboard-supported application, they will be required to agree to the application Privacy Policy and to the Terms and Conditions (presented separately within the respective app). By agreeing to the Privacy Policy, users are providing consent for their Personal Health Information to be stored and used by OneDashboard as described below.
Data Collection & Usage
The following information is collected and securely stored within the OneDashboard.
Clinician/Researcher Account Setup
To create a OneDashboard account for clinicians or research staff, we collect the following information:
Name (first and last)
Associated clinic or research program
Email address
This information is used solely to create and manage your OneDashboard account, enable secure login, and provide necessary communications related to system use.
Patient Profile Setup
When adding a patient to OneDashboard, the following information may be collected:
First Name (required)
Middle Name
Last Name (required)
Medical Record Number (MRN)
Health Card Number (HCN)
Version Code
Date of Birth (DOB)
Sex
Primary and Secondary Phone Numbers
Email Address
Home Address (Street, City, Province, Postal Code)
Clinic Name (required)
Most Responsible Physician (MRP)
Status (e.g., Active)
Diagnosis
First Name, Last Name, and Clinic are the only required fields. This information is required to create a patient profile within OneDashboard and ensure accurate linkage to their clinical team. All other information is not required and may be collected by OneDashboard clinicians/researchers at the patient's discretion.
Caregiver Information
If caregivers are added to a patient’s profile, the following details may be collected:
First Name (required)
Middle Name
Last Name (required)
Relationship to Patient
Does the caregiver live with the patient? (Yes/No)
Phone Number
Email Address
First Name and Last Name are the only required fields. This information is required to document a caregiver within a patient profile on OneDashboard. All other information is not required and may be collected by OneDashboard clinicians/researchers at the caregiver's discretion.
Application Account Invitation
Patients and caregivers may be invited to create accounts for supported applications integrated with OneDashboard. In order to invite a user a name and email address are required.
Automatically Collected Information
The OneDashboard collects certain information from you automatically, which is limited to analytics around the OneDashboard screens you use. This automatically collected data will only be used by the administrative team within UHN responsible for maintaining the OneDashboard, and for hosting the data submitted to the OneDashboard, and may include third party agents of UHN, acting solely on UHN’s behalf and will only be used for system administration purposes, where access to such data will be limited to diagnosing problems with servers (such as software bugs), if any; to monitor traffic patterns; to determine which services are used most frequently; to assess site usage; to help to improve the usability of the OneDashboard.
By creating an account with the OneDashboard, you are agreeing to the automatic collection of the data listed above, solely for use by the OneDashboard Team.
In addition to the use of the Automatically Collected Data described above, the OneDashboard Team may also use information provided by you in response to the patient-reported symptom survey that has been stripped of all directly identifying personal information (such as your name, email address and other information which directly identifies you) for the purposes of improving the quality of the OneDashboard.
Cookies
The OneDashboard may use “cookies” for the convenience of users and to enhance your experience using this OneDashboard. A cookie is a small text file that is recorded either temporarily or persistently on your computer by a web page server and is not intended to deliver viruses or other harmful programs to your computer.
On the OneDashboard, cookies are used primarily for authentication and security purposes:
Cookies are used to avoid signing into your account repeatedly with 1+ authentication within a period of 30 days. By using cookies, OneDashboard is able to deliver a more efficient service that is more personalized for each user.
Web browsers generally accept cookies automatically. When you sign in to the OneDashboard, you can:

How We Share Your Data
With Third Parties
We will not sell, rent, or share your Personal Health Information with third parties other than as outlined herein.
We may disclose your Personal Health Information as required by law, such as to comply with a subpoena, or similar legal process.
How You Can Control Your Data
You will have control over your Personal (Health) Information. If at any point you identify information that needs to be corrected or removed, you can contact the OneDashboard Team by email at onedashboardsupport@uhn.ca. Patient/Caregiver data is controlled by the Patient/Caregiver by contacting the OneDashboard user, who will then redirect the request to the technical team.
How We Store Your Data
All information gathered from you will be stored and processed in Ontario, Canada on University Health Network servers. If you access your OneDashboard account from a location other than Canada, your information will be transferred to Canada. Ontario and Canadian law are different from that of other jurisdictions. Ontario and Canadian laws apply to the use, storage, transfer, management and handling of your data.
The OneDashboard may retain patient PHI for as long as they use the application. Users may submit a request with the OneDashboard user to discontinue their participation in the OneDashboard-supported application, subsequently the collection of any new Personal Health Information in the application will be stopped. Historical Personal Health Information in OneDashboard will be handled in accordance with this Privacy Policy. For patient data, OneDashboard will work in accordance with integrated application privacy policy.
Records will be retained for at least ten years.
How We Keep Your Data Secure
The OneDashboard has implemented security measures intended to protect yours, patient, and caregiver's Personal Health Information both online and offline, including measures intended to restrict unauthorized persons from accessing Personal Health Information. Encryption technologies are used when transferring and receiving data provided through the OneDashboard. We also have security measures in place at our physical facilities to protect against the loss, misuse, or alteration of Personal Health Information stored in the OneDashboard. We also have restricted access to the servers to the OneDashboard team, and have the ability to audit who accesses your data and when. Although we have implemented various measures to protect your information, the OneDashboard does not and cannot guarantee absolute security of any information.
We are concerned about safeguarding the confidentiality of your information. We provide physical, electronic, and procedural safeguards to protect information we process and maintain. For example, we limit access to this information to authorized employees and contractors who need to know that information in order to operate, develop or improve our OneDashboard.
Theft, Loss or Unauthorized Access of Your Information
In the event that UHN becomes aware that your, patient, or caregiver Personal Health Information submitted through the OneDashboard has been stolen or lost, a person has obtained unauthorized access to it, or someone has used, disclosed or disposed of in in a manner other than as described in this policy, UHN will investigate the incident and notify you if a breach has occurred. If you become aware of any unauthorized use of your Personal Health Information submitted through this OneDashboard, please immediately notify UHN Privacy Office at the address below.
Modifications and Updates to the Privacy Policy
This Privacy Policy may be updated from time to time for any reason. Please also check this Privacy Policy regularly for any changes.
